Address Changes
*Address must match respective resident/home state license address
- P.O. Boxes are not permitted in the applicant’s residence or business address fields.
- Applicant’s OK license record may be in active or inactive status on PDB.
- Individual applicants will not be allowed to change the state code of their residence address for their resident license record.
- Applicants will be allowed to change the state code of their business and/or mailing addresses for their resident license record.
Name Changes
- Business Entities: All documentation must be promptly uploaded to the Attachment Warehouse within 24 hours of transaction submission. Failure to comply will result in transaction denial, necessitating resubmission.
- Domicile businesses are obligated to contact the Oklahoma Secretary of State, while foreign businesses must provide either a Certificate of Authority or a compliance letter from their Secretary of State.

- Documentation is required to be uploaded to the Attachment Warehouse for Individual Name Change. Documentation that will be accepted: Social Security Card, Marriage Certificate, Divorce Decree, Certified Court document, or Driver’s License. The date on your provided document will be the effective date used to determine if you owe a fee with your name change request. If a fee is owed, an invoice will be sent from the Oklahoma Insurance Department.
