Fees shown are state fees only. These fees do not include NIPR transaction fees.
- State Fee: $0
- Address Violation Fee: $50
- Applied if the requested effective date is greater than thirty (30) days or applicant has an address violation.

Applicant Can
- Submit name and contact change requests for any license on NIPR's Producer Database (PDB).
- Submit contact change request for any license on NIPR's Producer Database (PDB).
- Submit DHS contact change request (no resident state license on PDB).
- Tracks counties.
- Submit Email Changes.
- Submit Phone number Changes.
- Accepts change to state code of business address (Resident Licensees).
- Accepts change to state code of mailing address (Resident Licensees).
- *Accepts change to state code of residence address (Non-Resident Licensees).
- *Accepts change to state code of business address (Non-Resident Licensees).
- *Accepts change to state code of mailing address (Non-Resident Licensees).
Applicant Cannot
- Submit change to state code of residence address (Resident Licensees).
Business Rules
Address Changes
*Address must match respective resident/home state license address.
- P.O. Boxes are not permitted in the applicant’s residence or business address fields.
- Applicant’s OK license record may be in active or inactive status on PDB.
- Individual applicants will not be allowed to change the state code of their residence address for their resident license record.
- Applicants will be allowed to change the state code of their business and/or mailing addresses for their resident license record.
Name Changes
- Business Entities: All documentation must be promptly uploaded to the Attachment Warehouse within 24 hours of transaction submission. Failure to comply will result in transaction denial, necessitating resubmission.
- Documentation is required to be uploaded to the Attachment Warehouse for Business Entity Name Change. Businesses must provide a Certified Amendment/Conversion documents from the state of domestic incorporation showing the date of change.
- Documentation is required to be uploaded to the Attachment Warehouse for Business Entity Name Change. Businesses must provide a Certified Amendment/Conversion documents from the state of domestic incorporation showing the date of change.
- Individuals: All documentation must be promptly uploaded to the Attachment Warehouse within 24 hours of transaction submission. Failure to comply will result in transaction denial, necessitating resubmission.
- Documentation is required to be uploaded to the Attachment Warehouse for Individual Name Change. Documentation that will be accepted: Social Security Card, Marriage Certificate, Divorce Decree, Certified Court document, or Driver’s License. The date on your provided document will be the effective date used to determine if you owe a fee with your name change request. If a fee is owed, an invoice will be sent from the Oklahoma Insurance Department.
