Applicants must provide a valid Hawaii residence and business address. Address may not contain a P.O. Box

A licensee shall apply to the Department of Commerce and Consumer Affairs (”DCCA”) and the Insurance Commissioner for approval of the use or change of a trade name.

Licensees must have a General Excise Tax License, as required under the tax laws for the State of Hawaii. Please contact the state Department of Taxation for more information.
- Fingerprinting is required for resident license applications, effective November 1st, 2010:
- Applicants are required to be fingerprinted prior to submitting a license application for initial license or license reactivation.
- To obtain fingerprints, contact our fingerprinting vendor, FieldPrint, to schedule an appointment. All applicants must enter “HI-DCCA-INS”, the code that identifies the Insurance Division as the intended recipient of your fingerprint result, when scheduling an online appointment with Fieldprint. Fees for fingerprinting (including state and federal submission fees) are collected by FieldPrint.
- Voluntary Surrender applicants who are reinstating within one year are exempt from fingerprint requirement.
- Reinsurance Intermediary Brokers and Reinsurance Intermediary Managers, Life Settlement Brokers and Managing General agents must actively hold an Insurance Producer license. They can only apply for the reciprocal LOA they maintain on their Insurance Producer License.

All additional documents are required to be attached to the NIPR Attachment Warehouse within 7 days of submitting the application.
- Insurance Producer and Surplus Lines applicants must successfully pass the Hawaii Insurance Producer licensing exam(s). Hawaii does not allow any professional insurance designations as exemptions to licensing examinations.
- An appointment form does not need to be submitted with the application. However, to §431:9A-114 Appointments: (a) an Insurance Producer shall not act as an agent of an insurer unless the Insurance Producer becomes an appointed agent of that insurer. All Producers will need to be appointed directly through an Insurance Company and appointment termination can only be submitted by an Insurance Company.

Variable applicants must supply HAWAII FINRA CRD number on application. If you need to verify your CRD number you may use the Broker Search feature found on FINRA’s website. Applicants must verify that they are registered in Hawaii with FINRA before they can apply for Variable. Applicants are required to register for Broker “Agent” with FINRA in Hawaii.
- Third Party Administrator applicants are required to:
- All applications for Third Party Administrator will defer to the state for review. Go to http://cca.hawaii.gov/ins/producers/forms/ and open TPA CHECKLIST form. Check appropriate boxes on the form and complete the checklist to assure completion of application filing. A completed checklist must be accompanied with all documents required on the checklist and submitted through the Attachment Warehouse.
- Third Party Administrator applicants must enter a valid United States business and mailing address.
- Public Adjuster applicants are required to:
- Pass the Hawaii insurance adjuster examination for resident and non-resident applicants.
- Applications for Public Adjuster will defer to the state for review. Go to Hawaii Revised Statute §431:9-223 for bond requirements. Applicant must submit a valid $10,000 Surety Bond through the Attachment Warehouse.
- Workers’ Compensation Adjuster applicants are required to:
- Pass the Hawaii Workers Compensation Insurance Adjuster examination for resident and non-resident initial applications AND renewals.
- Crop Insurance Adjuster applicants are required to:
- Submit a valid, unexpired, Crop Adjuster Proficiency Program ("CAPP") card to the Attachment Warehouse for resident and non-resident initial applications AND renewals. There is no exam required for resident or non-resident applicants.
- Independent Adjuster applicants are required to:
- Pass the Hawaii insurance adjuster examination for resident and non-resident applicants.
- Surplus Lines Brokers (SLB) – In order for the license to be issued, the business entity must submit “Form List of SLB(s)”. Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form List of SLB(s)” via NIPR’s attachment Warehouse
- Independent Bill Reviewer (IBR) - In order for the license to be issued, the applicant must submit credentials as a Certified Professional Coder granted by the American Academy of Professional Coders or American Health Information Management Association via NIPR’s Attachment Warehouse.
- Life Settlement Broker (LSB) - Must be licensed as a resident insurance producer with a line of life authority in Hawaii for at least one year to be eligible to apply for a life settlement broker license. In order for the registration to be issued, the individual must submit Anti-Fraud form to the Attachments Warehouse.
- Life Settlement Provider (LSP) -In order for the license to be issued, the individual must submit “Anti-Fraud Plan” & “Detailed Plan of Operation” & “All forms and Advertisement (If applicable)”. The individual must submit Anti-Fraud form to the Attachments Warehouse.

If an applicant is not a US citizen, a copy of a valid work permit or permanent resident card must be submitted via NIPR's Attachments Warehouse.

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse.

Pharmacy Benefit Manager, Services Contract Provider and Vehicle Protect Product Warrantor do not need to submit supporting documents if you answer "Yes" to any background questions on the application.

Active Hawaii Resident Producer Converting to Hawaii Nonresident Producer:
- Go to http://cca.hawaii.gov/ins/producers/forms/ and submit Form AC
- Letter of Certification issued by your new home state showing that you are licensed as a Resident Producer.
- Go to http://cca.hawaii.gov/ins/producers/forms/ and submit : Form APPT
Active Hawaii Nonresident Producer Converting to Hawaii Resident Producer:
- If the National Producer Database (PDB) shows that you currently hold an active resident license in another state, a clearance letter from that state will be required.
- Successfully pass the Hawaii Insurance License Exam(s). Contact our exam administrator, Pearson Vue to register for an exam. Call toll free at 1-800-274-2608. View our Candidate Handbook at Pearson Vue's website. Exam is not required if application is received within ninety days of the cancellation of the applicant's previous resident license and if the producer was licensed in good standing.

- A complete paper application includes:
- Form HPL – Hawaii Application for Individual Insurance License. Or NAIC Uniform Application. Submit one with original signature.
