Applicant may amend license if they have an active license or line of authority (LOA). If license or LOA have a revoked status, the applicant must wait one year from revocation date to apply for any new line.

Applicants for the variable LOA must supply FINRA CRD number on application.

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.
