All applicants must be tax compliant and submit all required documentation to Guam BEFORE June 15 of each year. If documentation is not submitted timely, applicant’s renewal will be subject to delay. Please contact Guam Department of Revenue and Taxation directly for more information.

Applicants must renew Insurance Producer Life when renewing Insurance Producer Variable Life and Annuity.

Applicant must hold an active Broker license to be eligible to renew Surplus Lines Broker.

- NON-RESIDENT INDIVIDUAL INSURANCE PRODUCER:

- Copy of active license from state-of-domicile
- Certificate of Good Standing issued by the Insurance Commissioner of the licensed state-of-domicile.
- NON-RESIDENT INDIVIDUAL BROKER:
- Police Clearance issued by the state-of-domicile or equivalent report from the applicant’s municipality of non-criminal record or history.
- Form I-9 Tax Clearance - https://www.govguamdocs.com/revtax/docs/TAXCLEARANCEAPPLICATIONFORM.pdf
- Copy of active license from state-of-domicile.
- Certificate of Good Standing issued by the Insurance Commissioner of the licensed state-of-domicile.
- NON-RESIDENT INDIVIDUAL SURPLUS LINE BROKER:
- Police Clearance issued by the state-of-domicile or equivalent report from the applicant’s municipality of non-criminal record or history.
- Form I-9 Tax Clearance –
- Certificate of Good Standing issued by the Commissioner of state-of-domicile.
- Surety Bond of $2,000 (Original bond must be submitted and term of the bond must coincide with the term of license period).
- NON-RESIDENT INDIVIDUAL INSURANCE PRODUCER:
- Please submit supporting documents electronically via NIPR's Attachments Warehouse.
