The Intermediary (Producer) Individual license fee of $10.00 applies to applications that require exams, which include; Life, Accident & Health, Property, Casualty, Personal Lines, Credit, and Title. Exam vendor or Wisconsin may collect additional state licensing fees. This fee is per license application.

The Intermediary (Producer) Individual line of authority fee of $75.00 applies to applications for lines of authorities that do not require exams, such as; Variable Life/Variable Annuity, Travel, Crop, Surety, Legal Expense, and Attorney Title. Fee also applies to applicants that were licensed as a producer in another state and application date is within one (1) year of cancellation date of previous resident license. The fee will be assessed per line of authority.

Business Rules
- Applicants Please Read:
- Once your application has been submitted to the State of Wisconsin, you will have 90 days to submit any required information/requirements. Failure to supply the required documentation will cause your application to be CLOSED as EXPIRED. Fees are non-refundable.
- If you applied for a license type in error, you will need to submit a written request to our office requesting the withdrawal of the application. The application process and the withdrawal of the application represents an administrative expense for the agency and, therefore, the fees are non-refundable.
- If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.

Please note: Wisconsin now uses the National Producer Number (NPN) as the license number.

Resident addresses must be in Wisconsin and cannot be a PO Box. Individuals, who want to designate Wisconsin as their home state, must contact the state for further instruction.

If adding new lines of authority while license is in reinstatement period (late renewal), applicant must reinstate through RLR before amending.

If applying for new lines of authority but currently under the reinstatement period for other lines of authority, applicant will need to surrender those lines of authority first by sending a statement to ociagentlicensing@wisconsin.gov before the new lines of authority can be applied for. After the lines of authority are in 'Voluntary Surrender' status on the Producer Database (PDB), applicant will be able to use the Resident License (RL) application to apply for new lines.
- Intermediary Individual means an agent, producer or an insurance broker. Intermediary (Agent) Individual license on the Producer Database and license certificate will display as Intermediary (Producer) Individual on the online application to help Producers select the correct license class. These two license class names are the same.

Intermediary (Producer) Individuals must reapply more than one (1) year after license expiration date through Resident Licensing (RL) Application. Individual applicants must complete pre-licensing class, take an exam, and submit fingerprints to reapply for the license. Individuals applying for the same line of authority previously held must take the exam for agent who previously held the line of authority.
- Public Adjuster applicant must submit proof of a performance bond. The minimum requirement for the performance bond is $20,000. Applicant must send an original performance bond (signed and sealed) directly to the Wisconsin Office of the Commissioner of Insurance via regular mail to the following address:

- Public Adjuster applicants: Section 629.02 (2) (d), Wis. Stat, in order to be licensed in Wisconsin, individual needs to maintain a home office in the individual’s state of residence and be available to the public at that office through reasonable appointment or regular business hours.

- - Address of home office in state of residence
- Employee Benefit Plan Administrator applicants must submit the following to the state:
- Proof of a new original Performance Bond (signed & sealed) between $15,000.00 and $500,000.00.
- Financial Statements for the administrator's most recent completed fiscal year. (balance sheet & income statement).
- Applicants must mail original performance bond directly to the state. All other documentation may be submitted via email at ociagentlicensing@wisconsin.gov.
- Life Settlement (Viatical Settlement) applicants must submit the following to the state:
- An original endorsement from a licensed insurer identifying professional liability insurance held in an amount not less than $1 million total annual aggregate.
- A list of licensed providers and provide the name and addresses of companies the broker will be representing.
- Provide evidence of completion of the life settlement 8-hour training requirement
- Veterans only – provide a copy of fee waiver voucher issued by the Wisconsin Department of Veterans Affairs (WDVA)
- Applicants can e-mail the documentation to the state.

Managing General Agent (MGA) Individual applicants must actively hold a resident Wisconsin Intermediary (Producer) Individual license on the Producer Database.

Property and Casualty applicants cannot apply for Personal Lines P&C. This authority is included in the major lines of Property and Casualty.

Personal Lines P&C applicants may apply for Property and Casualty lines of authority.

Surplus Lines applicants must actively hold a resident Wisconsin Intermediary (Producer) Individual license with Property and Casualty lines.

Variable Life/Variable Annuity applicants must actively hold or be applying for Life line of authority simultaneously. These applicants must also supply a FINRA CRD number on application. Intermediary (Producer) Individual is exempt from a license exam, pre-licensing, and fingerprinting if their previous resident license from another state has been inactive for less than one year. Individual only need apply for the license of the previously held lines of authority and pay the licensing fee.
- Note: Previous resident license must be inactivated prior to applying to Wisconsin and license must continue to display as an resident license on PDB in order to be exempt from these requirements. All Intermediary (Producer) Individual applicants must have a passed fingerprint check for the following lines of authority:

The passed fingerprint check is valid for 180 days. This check is applicable to new licenses, reapplications and amendments. If applicant has NOT submitted fingerprints yet, please make a reservation through Fieldprint at www.fieldprintwisconsin.com. At the time of reservation, applicants must enter Fieldprint code: FPWIOCIInsurance.
- All applicants must answer the following non-uniform questions:
- "Have you obtained a waiver by the Wisconsin Department of Veteran Affairs?"
- If question is answered in the affirmative, applicant must answer the follow-up question:
- "If yes, provide voucher number in space below that was issued through the Wisconsin Department of Veteran Affairs."

Note: The non-uniform questions listed above will not be asked on reapplications, amendments, or if the applicant is already licensed in Wisconsin. The voucher for this waiver is a one-time event only.
- Navigator Individual applicants must complete any federally mandated training required under the federal health care exchange. Send certificate of completion to ociagentlicensing@wisconsin.gov.
- Risk Purchasing Group and Rate Service Organization applicants will be considered reapplication following the expiration date.
