Applicant's name, Tax ID, license number, and state of residence must match current Producer Database (PDB) record for resident state
Applicant must not have a status of suspended, tax compliance, revoked, or tax compliance.
Utilization Review applications must be submitted by the Owner, Partner, Officer or Director.

- A written statement summarizing the details of each incident.
- A copy of the petition, complaint or other document that commenced the lawsuit, arbitration, or mediation proceedings.
- A copy of the official document, which demonstrates the resolution of the charges or any final judgment.
