A licensee shall apply to the Department of Commerce and Consumer Affairs (”DCCA”) and the Insurance Commissioner for approval of the use or change of a trade name.

An appointment form does not need to be submitted with the application. However, to §431:9A-114 Appointments: (a) an insurance producer shall not act as an agent of an insurer unless the insurance producer becomes an appointed agent of that insurer. All Producers will need to be appointed directly through an Insurance Company and appointment termination can only be submitted by an Insurance Company.

Licensees must have a General Excise Tax License, as required under the tax laws for the State of Hawaii. Please contact the state Department of Taxation for more information.

Applicants must provide a valid Hawaii resident and business address. Address may not contain a P.O. Box.
- Applicants are required to send the following attachments to the Hawaii Department of Commerce and Consumer Affairs, Insurance Division, Licensing Branch.
- Any licensed insurance producer so designated or empowered by a corporation or partnership may not be so designated or empowered by more than one corporation or partnership, except when the corporation or partnerships are affiliates of each other (must submit proof of affiliation such as organization chart or a copy of the articles of incorporation or articles of organization and identify the relevant page/portions of the document for ease of reference via NIPR's Attachments Warehouse). As used herein, a corporation or partnership is an affiliate of another corporation or partnership, if the same person directly or indirectly through one or more intermediaries, controls either corporation or partnership.

All listed Designated Responsible Licensed Producers (DRLPs) must cumulatively cover the lines of authority (LOAs) on the business application.

When applying for surplus lines Broker license, applicant must only apply for Casualty and Property.

Pharmacy Benefit Manager, Services Contract Provider and Vehicle Protect Product Warrantor do not need to submit supporting documents if you answer "Yes" to any background questions on the application.

When applying for surplus lines broker business entity license, applicant must submit a list of individuals (names and license numbers) within the business entity who are licensed Surplus Lines Brokers via NIPR's Attachments Warehouse. Only licensed individual Surplus Lines Brokers within your licensed business entity may transact surplus lines business. You must provide the Licensing Branch with the names and license numbers of the individual Surplus Lines Brokers who are added or removed from your business.
- Reinsurance Intermediary Brokers and Reinsurance Intermediary Managers, Life Settlement Brokers and Managing General agents must actively hold an Insurance Producer license. They can only apply for the reciprocal LOA they maintain on their Insurance Producer License.
- Third Party Administrator applicants are required to:
- All applications for Third Party Administrator will defer to the state for review. Go to http://cca.hawaii.gov/ins/producers/forms/ and open TPA CHECKLIST form. Check appropriate boxes on the form and complete the checklist to assure completion of application filing. A completed checklist must be accompanied with all documents required on the checklist and submitted through the Attachment Warehouse.
- Third Party Administrator applicants must enter a valid United States business and mailing address.
- Limited Lines Self-Service Storage Producer applicants are required to:
- Business Limited Lines Self-Service Storage Producer will defer to the state for review. Submit name, Hawaii resident address, telephone number and email of a designated responsible person who is responsible for the agency’s compliance with Hawaii insurance code via NIPR’s Attachments Warehouse.

License Voluntary Reinstatement
- Licenses are subject to reinstatement if they are within one year of surrendering license two years. Those wishing to reactivate their license should email the Hawaii Division of Insurance at inslic@dcca.hawaii.go
- Pharmacy Benefit Manager (PBM) – In order for the registration to be issued, the business entity must submit “Form DP for PBM” Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form DP for PBM” via NIPR’s attachment Warehouse
- Life Settlement Broker - Must be licensed as a resident insurance producer with a line of life authority in Hawaii for at least one year to be eligible to apply for a life settlement broker license.
- Reinsurance Intermediary Broker (RIB) and Reinsurance Intermediary Manager (RIM) – In order for the license to be issued, the business entity must submit “Form List of RI(s)”. Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form List of RI(s)” via NIPR’s attachment Warehouse
- Limited Lines Self-Service Storage Producer (SSS) – In order for the license to be issued, the business entity must submit “Form DP for SSS” Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form DP for SSS” via NIPR’s attachment Warehouse

Life Settlement Broker (LSB) - Must be licensed as a resident insurance producer with a line of life authority in Hawaii for at least one year to be eligible to apply for a life settlement broker license. In order for the registration to be issued, the individual must submit Anti-Fraud form to the Attachments Warehouse.
- Life Settlement Provider (LSP) -In order for the license to be issued, the individual must submit “Anti-Fraud Plan” & “Detailed Plan of Operation” & “All forms and Advertisement (If applicable)”. The individual must submit Anti-Fraud form to the Attachments Warehouse.

Active Hawaii Resident Producer Converting to Hawaii Nonresident Producer:
- Go to http://cca.hawaii.gov/ins/producers/forms/ and submit Form AC
- Letter of Certification issued by your new home state showing that you are licensed as a Resident Producer.
- Go to http://cca.hawaii.gov/ins/producers/forms/ and submit : Form APPT
Active Hawaii Nonresident Producer Converting to Hawaii Resident Producer:
- If the National Producer Database (PDB) shows that you currently hold an active resident license in another state, a clearance letter from that state will be required.
- Successfully pass the Hawaii Insurance License Exam(s). Contact our exam administrator, Pearson Vue to register for an exam. Call toll free at 1-800-274-2608. View our Candidate Handbook at Pearson Vue's website. Exam is not required if application is received within ninety days of the cancellation of the applicant's previous resident license and if the producer was licensed in good standing.
- Obtain fingerprints. See instructions
- A complete paper application includes:
- Form HPL – Hawaii Application for Individual Insurance License. Or NAIC Uniform Application. Submit one with original signature.
- Licensing Fees. Make check payable to DEPARTMENT OF COMMERCE AND CONSUMER AFFAIRS.
