Supporting Documentation Requirements
- Do not submit an application unless all required supporting documentation is ready to be provided.
- If an applicant responds “yes” to any background question or is otherwise required to submit additional documentation, those documents must be submitted as part of the application process.
- Preferred Submission Method
Supporting documentation should be uploaded electronically via NIPR’s Attachment Warehouse. Because access to the Attachment Warehouse is only available after an application is submitted, applicants should be prepared to upload all required documentation immediately upon submission. - Alternate Submission Methods
Supporting documents may also be submitted directly to the Louisiana Department of Insurance (LDI) via mail, email, or fax. If documentation is submitted using one of these methods: - Documents must be submitted the same day the license application is filed
- Documentation must include the application transaction number and applicant’s name
- Documentation submitted by mail must be postmarked within 24 hours of application submission
- Important: Applications submitted without complete supporting documentation may be closed as incomplete without further notice and may require reapplication.
- All required documentation must be received within thirty (30) days of application submission, or the application will be declined.
At least one (1) Designated Responsible Licensed Producer (DRLP) must be provided on the application and have an active resident or non-resident license in the state of Louisiana - excluding the Portable Electronics and the Self-Storage lines of authority (LOA).
DRLPs provided must cumulatively cover the LOAs on the application.
Please confirm through Producer Database (PDB) that each Designated Responsible Licensed Producer (DRLP) does hold an active license in Louisiana.
- If the DRLPs are not already licensed in Louisiana, the business entity license application will be declined.
- DRLP does not need to be licensed in Louisiana on applications for Portable Electronics and Self Storage, but must be listed on the application.
Upon license issuance and renewal, all Claims Adjusters licensed in Louisiana are required to read and acknowledge the claims adjuster standards of conduct provided in La. R.S. 22:1674.1 Your acknowledgment will be captured in a Louisiana state-specific question in the initial and renewal application submission. You may read review the standards of conduct on the Louisiana Department of Insurance website.
Applicant must provide a valid business address within the state of Louisiana. Address may not contain P.O. Box.
Applicants for the Variable LOA must supply FINRA CRD number on application.
Before the Portable Electronics authority will be issued, a list of Louisiana business locations must be submitted to the state Department of Insurance.
Before the Self-Storage authority will be issued, an Insurer Certification must be uploaded to the NIPR Attachment Warehouse or submitted to the state Department of Insurance.
Applicants for Public Adjuster Agency must submit an original surety bond to the Louisiana Department of Insurance. The bond form is available on the Louisiana DOI website.
- Note that each notarized signature requires two witnesses. The notary cannot be a witness.
- Incomplete bonds will not be processed and will not be returned.
Applicants for the Title authority have special requirements. Please review the Louisiana DOI website for more information and the required affidavit. The affidavit can be uploaded to the NIPR Attachment Warehouse.
Reapplication:
