At least one (1) Designated Responsible License Producer must be on the application.

Surplus lines applicants must enter a designated responsible producer that actively holds a surplus lines license in Indiana.

Self-storage applicants must enter at least one (1) designated responsible producer that actively holds a resident producer license with ‘property & casualty’ line of authority.

Portable electronics applicants must enter at least one (1) designated responsible producer that actively holds a portable electronics limited lines license class and/or a resident producer license with ‘property & casualty’ line of authority.

The designated responsible producers must cumulatively cover the lines of authority on the business application. This excludes self-storage insurance, surplus lines and portable electronics.

Applicant must not apply for nor actively hold both ‘Life, Accident & Health’ line of authority and single authorities of ‘Life’ and ‘Accident & Health’

A resident independent adjuster applicant may not apply if the applicant holds an active Indiana resident public adjuster license.

One year after expiration, applicant must apply as new.

Newly licensed surplus lines producers have their renewal month synchronized with their producer license renewal month.

Applicant's producer license with property & casualty line of authority must be active to apply for the surplus lines license.

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax. All other required documentation must be forwarded directly to the state.

Applicants applying for an Indiana Resident License that have an active resident license in another state on the Producer Database (PDB) will be allowed to apply but will have their transactions defer.
