- Submit initial license for individuals electronically.
Business Rules
If applicant responds "yes" to one of the background questions, please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.
Applicants must not have an active resident state license on PDB in any state.
At least one Owner, Partner, Officer or Director (OPOD) must be entered on the application.
Business Entity applicants applying for Public Adjuster – all entries on the Owner, Partner, Officer and Director (OPOD) section of the application must be actively licensed as a Public Adjuster on the Producer Database (PDB).
Business Entity applicants must supply a business address that is not in Illinois. If business address supplied is in Illinois, the transaction will not be allowed.
Applicants applying for a Pharmacy Benefit Manager license must submit the address(s) of each Owner, and/or Director supplied on the application to the state directly. Please email to DOI.Licensing@illinois.gov.
- All business entity applicants must enter at least one Designated Responsible Licensed Producer (DRLP). Exclusion – the following license classes do not require a DRLP to be entered and can be left blank: Pharmacy Benefit Manager, Portable Electronics Limited line license, Premium Finance Company, Third Party Administrator, Third Party Prescription Program, Employee Leasing Company, Self Service Storage Facility.
- Each Designated Responsible Licensed Producer (DRLP) entered on the application must be actively licensed in Illinois. Unlicensed DRLPs are not permitted.
- Business Entity applicants applying for Viatical Settlement Broker must enter a Designated Responsible Licensed Producer (DRLP) that is actively licensed as a Viatical Settlement Broker on PDB.
- Business Entity applicants applying for Public Adjuster must enter a Designated Responsible Licensed Producer (DRLP) that is actively licensed as a Public Adjuster on PDB.
- At least one of the Designated Producers specified on the application must also be specified as an Owner, Partner, Officer or Director. Note: this excludes offerings that do not require a DRLP to be supplied.
Applicants are allowed to reapply immediately following the license expiration date. There is no late renewal period.
P.O. Box or PMB (Private Mailbox) cannot be used in the business address field.
Public Adjuster applicants are required to submit a bond in the amount of $20,000. Applicant is required to upload required documentation to the state via the NIPR Attachment Warehouse.
