Applicant Can

Business Rules

A licensee shall apply to the Department of Commerce and Consumer Affairs (”DCCA”) and the Insurance Commissioner for approval of the use or change of a trade name.
All phone and fax numbers entered on application, including authorized submitter information, must contain ten (10) digits.
Applicant must provide a valid business email address and business phone number.
Applicant must renew all active lines held at renewal. No partial renewals are allowed.
When renewing surplus line broker license, applicant must only renew casualty and property.
If license went inactive before the expiration date, the applicant must contact the Hawaii Division of Insurance.
At least one (1) Designated Responsible License insurance producer (DRLP) must be provided on the application and have an active resident or non-resident license in Hawaii.
The DRLPs provided must cumulatively cover the LOAs on the application.
- Third Party Administrator applicants are required to:
- All applications for Third Party Administrator will defer to the state for review. Go to http://cca.hawaii.gov/ins/producers/forms/ and open TPA CHECKLIST form. Check appropriate boxes on the form and complete the checklist to assure completion of application filing. A completed checklist must be accompanied with all documents required on the checklist and submitted through the Attachment Warehouse.
- Third Party Administrator applicants must enter a valid United States business and mailing address.
Service Contract Provider (SCP) – In order for the registration to be issued, the business entity must submit “Form DP for SCP” and “Form FRS”. Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form DP for SCP” and “Form FRS” via NIPR’s attachment Warehouse.
Vehicle Protection Product Warrantor (VPPW) – In order for the registration to be issued, the business entity must submit “Form DP for VPPW” and “Form FSS”. Go to https://cca.hawaii.gov/ins/producers/forms/ find and submit the “Form DP for VPPW” and “Form FSS” via NIPR’s attachment Warehouse.
Life Settlement Broker applicants must be actively licensed for ‘Life’ line of authority (LOA) in the resident/home state.
- Applicants renewing for the following license classes must hold an active Insurance Producer license in Hawaii.
