To report a legal name change for an individual or entity:

• Transitioning from nonresident to resident status: Individuals who apply within 90 days of establishing residency or a principal place of business in Ohio may be exempt from Ohio’s pre-licensing education and examination requirements if their current or previous resident license is in good standing. If the previous license was canceled, the Ohio application must be received within 90 days of its cancellation. Applicants that currently hold a nonresident OH license and want to apply through NIPR must first contact ODI staff to inactive their nonresident license.

• Transitioning from resident to nonresident status: Submit Form INS3241 within 30 days after changing your home state. Additionally, provide the Ohio Department of Insurance (ODI) with verification of licensure from your new home state. If your Ohio license is in good standing, ODI will convert it to a nonresident license without requiring a new application or fee. To request a clearance letter from Ohio, submit Form INS3254.

ODI may verify an applicant’s prior licensing status through the Producer Database. If the information is unavailable, ODI may request documentation from the applicant’s prior home state.
