License management

Apply for a license

To apply for a license in Ohio, visit LicenseHub.
If you can’t apply for the lines for which you have qualified, contact the state to verify that they have received the exams and any necessary pre-licensing education completion information. Also ensure that they have reported this to NIPR with the correct last name, social security number and date of birth. 

Renew a license

To renew a license in Ohio, visit LicenseHub.

If you can’t renew a particular line offered via NIPR, verify that the line is actively held in your resident state. If the line is not held, you cannot request that line via NIPR.

Visit the Ohio Department of Insurance (ODI) Agent Licensing page on their website.  

Cancel or surrender a line of authority

To cancel or surrender a line of authority: 

  • Entity: Send a signed letter from the owner or officer directly to the Ohio ODI. The letter should include the entity name, entity license number, and full name and signature of the owner or officer. The letter should be sent using the contact information listed below. 

Information updates

Report a legal name change – individual or entity

To report a legal name change for an individual or entity:  
Individual: Complete the ODI Individual Agent Address/Name Change Request form. Follow the submission instructions included on the form. 

Entity: Use the ODI Notification of Changes for Business Entity online form. To submit the form, click Submit Form at the bottom of the page. A printable Notification of Changes for Business Entity CN-65 form is also available at that link if you want to complete the form manually. Mail the completed document to the address on the form.  

Update contact information – individual or entity

To update contact information for insurance professionals:  

 Individual: Complete the ODI Individual Agent Address/Name Change Request form. Follow the submission instructions included on the form.

Report changes to Affiliations or Designated Responsible Licensed Producers

Submit changes to affiliations or Designated Responsible Licensed Producers (DRLPs) using the ODI Notification of Changes for Business Entity online form. To submit the online form, click Submit Form at the bottom of the page. There is also a printable form available at that link, if you want to complete the form manually. Mail the completed document to the address on the form.

Report changes to Owners, Officers, Partners or Directors

Report changes to Owners, Partners, Officers or Directors using the ODI Notification of Changes for Business Entity online form. To submit the online form, click Submit Form at the bottom of the page. A printable Notification of Changes for Business Entity CN-65 form is also available at that link if you want to complete form out manually. Mail the completed document to the address on the form.

Understand residency type changes 

Make ODI aware of residency changes using the Individual Agent Address Name Change Request form. Follow the submission instructions included on the form.
