License management
Apply for a license
To apply for a license in Arizona, visit LicenseHub.
If you can’t apply for the lines for which you have qualified, contact the state to verify that they have received the exams and any necessary pre-licensing education completion information. Also ensure that they have reported this to NIPR with the correct last name, social security number and date of birth.
Renew a license
To renew a license in Arizona, visit LicenseHub.
If you can’t renew a particular line offered via NIPR, verify that the line is actively held in your resident state. If the line is not held, you cannot request that line via NIPR.

Cancel or surrender a line of authority
To surrender a license or delete a line of authority, complete the Voluntary Surrender of Insurance License form. Submit the completed form to the email or mailing address listed on the form.

To update a Designated Responsible Licensed Producer (DRLP), visit LicenseHub.
Report changes to Owners, Officers, Partners or Directors
To report changes to Owners, Officers, Partners or Directors, complete the Change to Members, Officers or Directors (Principals) form. Print and mail your completed form to the address listed at the bottom of the second page.
