Applicant Can
- Submit name changes electronically.
- Submit DHS contact change request (no resident state license on PDB).
- Submit email changes.
- Submit phone number changes.
- Submit change to state code of residence address (Non-Resident Only).
- Submit change to state code of business address (Non-Resident Only).
- Submit change to state code of mailing address.
Applicant Cannot
- Submit change to state code of residence address (Resident Only).
- Submit change to state code of business address (Residents Only).
Business Rules
P.O. Box not allowed for residence and business address.
P.O. Box allowed for mailing address.
Applicant’s South Dakota license record may be in any status on PDB.
Non-resident South Dakota individual license: Applicant’s residence and/or business address must not be in South Dakota.
Non-resident South Dakota business entity licensee: Applicant’s business address must not be in the state of South Dakota.
Name Changes
Resident South Dakota Individuals: Resident name changes need to be verified by the South Dakota Division of Insurance. Documentation is required to be uploaded to the Attachment Warehouse for Individual Name Change. Documentation that will be accepted; Marriage Certificate, Divorce decree, Certified Court document, and/ or driver’s license.
Resident South Dakota Business Entities: Resident name changes need to be verified by the South Dakota Division of Insurance. Documentation is required to be uploaded to the Attachment Warehouse. Domicile businesses are required to contact the South Dakota Secretary of State at https://sdsos.gov/
