License management
Apply for a license
To apply for a license in Utah, visit LicenseHub.
If you can’t apply for the lines for which you have qualified, contact the state to verify that they have received the exams and any necessary pre-licensing education completion information. Also ensure that they have reported this to NIPR with the correct last name, social security number and date of birth.
Renew a license
To renew a license in Utah, visit LicenseHub.
If you can’t renew a particular line offered via NIPR, verify that the line is actively held in your resident state. If the line is not held, you cannot request that line via NIPR.
Print a license
To print your license, access your state’s license print tool and follow the instructions on the page.
Find current producer licensing information
Visit the Producers page on the Utah Insurance Department website.
Cancel or surrender a line of authority
To cancel or surrender a license, complete Section 4 of the Individual/Agency Change Request form.
Submit the form by fax to 385-465-6055 or email as a PDF to Licensing.UID@Utah.gov.
Information updates
Report a legal name change – individual or entity
Individual: Visit the Utah Producers portal. Select the Change an Individual Address, Name or Email Address option and follow the prompts. You’ll need to create a Producer Edge account if you don’t already have one.
Entity: Contact the state for guidance on entity name changes through their Reach Out to Us form.
To list an assumed name for an insurance professional (individual or entity), complete Section 2 of the Change Request form. Fax this form to 385-465-6055 or email it as an attachment to Licensing.UID@Utah.gov.

Update contact information – individual or entity
To update contact information for insurance professionals, visit the LicenseHub or the Utah Producers portal.
All changes must be reported within 30 days of the change. This includes home, business and mailing addresses, as well as phone numbers. Changes must be made by the license holder. This applies to both individuals and agencies.
NOTE: when changing a mailing or business address, you can use your residence address, but that information becomes public. Whenever possible, producers should use their business address as their mailing address. Business addresses, phone numbers, and fax numbers are public information, but residence information is not disclosed.
Report changes to Affiliations or Designated Responsible Licensed Producers
To report changes to affiliations or Designated Responsible Licensed Producers (DRLP) visit the Utah Producers portal. Select the Maintain Your Firm Association link and follow the prompts. You’ll need to create a Producer Edge account to follow this process, if you don’t already have one.
Report changes to Owners, Officers, Partners or Directors
Contact the state of Utah using the Reach Out to Us page on their website for information on changing Owners, Partners, Officers or Directors.
