Organizational Documents
 
 
Copy of fictitious name report from Oklahoma Secretary of State (if a trade name will also be used by the agency.)
Appointment Requirements
 
Company Appointment must be submitted within fifteen (15) days from the date the agency contract is executed or the first insurance application is submitted.
 
  The appointment fee is $40 per appointment (biennially).

Other Requirements

 
Attach cashiers check, business check, personal check or money order with new applications in the amount of a $20.00 for a Designation of Insurance Commissioner as Agent for Service of Process fee and a $20 application review fee.. The application fee is $60.00, unless, retaliatory home state fee is greater. Include an additional $60.00 fee for variable annuity. Make check payable to the Oklahoma Insurance Department.
 

"DBA" names will not be licensed.  Name cannot be misleading per 36 O.S. § 620(B)*

 
The business entity must designate a licensed producer responsible for the business entity's compliance with the insurance laws, rules and regulations of Oklahoma . 36 O.S. §1435.7(B)(2)
 
If applying for non-resident license for the first time, complete the Designation of Insurance Commissioner as Agent for Service of Process and submit the $20.00 fee and a $20.00 fee for the Application Review.

Company Appointment is not required with original application. However, an appointment is required to represent an insurance company and to receive commissions. The appointment must be submitted within fifteen (15) days from the date the agency contract is executed or the first insurance application is submitted.
 
The appointment fee is $40.00.
Other Requirements
 
Attach cashiers check, business check, personal check or money order for the appropriate fee of $20 SERVICE OF PROCESS fee and a $20.00 Application Review Fee. The resident fee is $60.00 and we are retaliatory. If the home state fee is greater, that fee will prevail. Make check payable to the Oklahoma Insurance Department.
 
Letter of Certification - Not required if information is available on the Producer Data Base (PDB). This may be verified with the home state insurance department.

Mail an original Secretary of State certified copy of the agency Articles of Incorporation (S-Corp or C-Corp) or Articles of Organization (LLC, LLP, LC or LP) to the Oklahoma Insurance Department with new agency applications for agency name changes.
 
Copy of trade name report from county clerk (if applying under a trade name)

Financial Responsibilities (brokers only)
 
Proof of Bond (Insurance Broker - $5,000 and Surplus Lines - a minimum of $5,000)
 
Proof of E & O (Insurance Broker - not less than $100,000)

License Fee is $60.00. Include an additional $60.00 for variable annuity
 
Oklahoma does not require that the designated/responsible producer be an officer, director or partner of the business entity.
