| Producer License Requirements |

Sign Application. Application must be signed by an owner, partner, member, officer or director of the business entity. |

Attach
a check or money order for $130. Must be made payable
to Treasurer, State of CT.
DESIGNATED RESPONSIBLE PRODUCER: Each Business Entity Producer application must name at least one Connecticut licensed producer, who will be responsible for the business entity’s compliance with the laws of the State of Connecticut. Such designee(s) must hold lines of authority equal to, or greater than, that of the business entity. |

| Registration of Assumed Name Requirements |
|
Assumed Names (Trade Names/DBA Names) using the same Tax Identification Number as the licensed Business Entity do not require licensure in Connecticut. Registration of the Assumed Name is required and can be accomplished by submitting the name information online at: https://www.cid-online.ct.gov/ There is no fee to register an Assumed Name.

No attachments required, unless there are "yes" answers to Background Information (page 3), and then follow instructions on page 3 of the application, regarding attachments. |

Appointment Requirements After Licensing

None required for producer licensure. Appointing Insurers must submit appointment requests in accordance with section 38a-702m of the Connecticut General Statutes. Appointments cannot be requested until an active Connecticut Producer license is in place. Notice of appointment must be filed with the Commissioner, not later than 15 days after the date the agency contract is executed or the first insurance application is submitted. |
