Individual insurance producer, Consultant-Individual and Viatical Broker applicants must pass the required exam for the requested line of authority (LOA).
Applications for Consultant-Individual license class require a history of 3 years licensed as an insurance producer with Life & Health or Property & Casualty line of authority (LOA) in Nebraska or another state.
If applying for Variable line the applicant must already hold or be applying for Life and Annuities line of authority (LOA) in Nebraska.
All individual Variable applicants must provide proof of Series 6 or 7 securities license held. Please fax documents to the state.
If applying for individual insurance producer – Surplus Lines license class, applicant must hold an active insurance producer license in Nebraska.
All applications for Motor Club must either take the Motor Club Exam or have an appointment from an underwriting Company.
The Motor Club Representative license is a perpetual license that does not renew or expire if the individual has an appointment on PDB.
All applicants for navigator must pass federal administered exam and provide certificate of proof and notify the state of the entity navigator with which he or she is affiliated and supervised. Please fax documents to the state.
Applicant applying for the insurance producer-PreNeed license class must list exactly one Business Entity affiliation. More than one affiliation to a business entity is NOT permitted.
All applications for managing general agent must provide proof surety bond to Nebraska Department of Insurance. Please fax documents to the state.
All applications for utilization review agent must provide proof of approval or accreditation by URAC or NCQUA to Nebraska Department of Insurance. Please fax documents to the state.
All applications for Funeral Insurance-Director LOA must provide proof of Director's certificate license held. Please fax documents to the state.
NE will allow reinstatement of a license through the resident licensing (RL) application with additional fee if license is 31days to one year past expiration date. Applicants must be CE compliant.
