Primary Rules (Individual)

License Printing

Business Rules (Individual)

Applicant's name, SSN/FEIN, license number, addresses, and state of residence must match current Producer Database (PDB) record for resident state.

Applications containing a non-uniform class or lines of authority (LOA) will be deferred to the state.

Applicant with status reason code of affidavit on file may not amend or reinstate.

Applicant for the variable LOAs must supply FINRA CRD number on application, and will be deferred pending FINRA verification.

Applicant applying for a surplus line broker license must actively hold both property and casualty LOAs.

To apply for the Life Agent Registered as Life Settlement Broker the agent must have held an active license in at another state within a year of Kentucky application.

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.

Business Rules (Business Entity)

Business entity applications will be reviewed by the state.

Applicant's name, FEIN, license number, addresses and state of residence must match current Producer Database (PDB) record for resident state.

Applications containing a non-uniform class or lines of authority (LOA) will be deferred to the state.

Applicants applying for surplus line broker licenses must actively hold both property and casualty LOA's.

Applicant cannot hold or apply for multiple active adjuster licenses.

All adjuster licenses will require either the minimum $1000.00 Adjuster Surety Bond (Form 99-3) or Adjuster Insurance Assumption (Form 99-6) for both residents and non-residents. The documents must be issued by a Kentucky admitted insurer and must contain original signatures. Copies of forms are not accepted. All adjusters are required to have an active surety bond on file as long as the license is active.

Each Designated Responsible Licensed Producer (DRLP) must have an active resident or non-resident license in Kentucky.

The DRLPs provided must cumulatively cover the LOAs on the application.
