- Submit change to state code of business address for non-residents. Address must match respective resident/home state license address.
- Submit change to state code of mailing address for non-residents. Address must match respective resident/home state license address.
- Submit change to state code of residence address for non-residents only. Address must match respective resident/home state license address.
- Submit change to state code of business address for residents. Individuals may change the state code only if the residence address state code is Indiana.
- Submit change to state code of mailing address for residents.

P.O. Boxes are allowed for mailing addresses.

P.O. Boxes are not allowed for residence addresses.

Indiana Residents must maintain a residence or business address in Indiana (for individuals).

Indiana Residents must maintain a business address in Indiana (for Business entities).

Indiana Non-Resident applicant's business address must not be in the state of Indiana (for Business entities).

If the business address is in Indiana and the residence address is not in Indiana, then the business address cannot be a P.O. Box (for individuals).

For Indiana residents, the applicant cannot change the state code from Indiana to another state for the business address if the residence address state code is not Indiana.

Applicant's Indiana license record may be in any status on PDB for a contact change request (CCR) transaction to be submitted.
