Arizona to convert to State Based Systems (SBS). Effective 6:00 p.m. CDT on Tuesday, April 11th, 2017, processing of all Arizona Gateway applications will be temporarily suspended. Gateway processing is expected to resume on Tuesday, April 18th, 2017.

 

NIPR will be reloading Arizona data for display on PDB during the SBS conversion. Arizona PDB data will be frozen as of Wednesday, April 12th 2017. PDB data will be converted and ready for display on Tuesday, April 18th, 2017

Business Rules (Individual)

Applicant trade names must be reported to the Department. Trade names should also be filed with the Arizona Secretary of State.

Applicant may not use a P.O. Box in the business or residence addresses.

Applicant must provide a valid residence phone number.

Applicants for the Variable line of authority (LOA) must supply FINRA CRD number on application.

Applicant cannot apply for ADJUSTER-LMTD TO RES AUTH and Adjuster LOA on the same application

If applicant actively holds an LOA of ADJUSTER-LMTD TO RES AUTH do not allow applicant to apply for Adjuster LOA

If applicant currently holds an LOA Adjuster do not allow applicant to apply for ADJUSTER-LMTD TO RES AUTH

Life Settlement Brokers must hold an active Life LOA on PDB or be applying for an underlying ‘Producer, Life’ line of authority at the same time as the Life Settlement Brokers LOA.

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.


Licensing Time Frames:
The Administrative Completeness Review Period (ACRP) for an application is one hundred and twenty (120) days pursuant to Arizona Revised Statutes Section 41-1071 et seq., and Arizona Administrative Code R20-6-708. The ACRP is the time within which the Arizona Department of Insurance must determine whether all required information has been submitted on the application. The ACRP is suspended if an application does not contain all the information required to be submitted. The online application may represent only a portion of information required for a complete license application. After submitting an electronic application, NIPR will send a confirmation e-mail informing that the license application has been accepted by Arizona or that additional information is required before the application can be accepted.

  • If the application is accepted by Arizona, the ACRP is deemed completed and the Department must make a substantive decision concerning the license application within sixty (60) days after the close of the ACRP. Generally, once a license application is accepted (and not deferred), a license is issued within a couple business days. Verify that a license has been issued by looking up information about the license on Arizona's state web site. The license certificate will be sent to the mailing address furnished on the application.
  • If additional information is required (for example, if the application is deferred), the ACRP will be immediately suspended, and the Insurance Licensing Section must receive a response to each specified deficiency or request for additional information within sixty (60) days after the date the e-mail was received from NIPR. Please include the Non-Resident License (NRL) transaction number in all written correspondence to the Department. If the Insurance Licensing Section does not receive a response to each specified deficiency or request for additional information within sixty (60) days, the application will be deemed withdrawn, the Department shall not be required to take any further action with respect to it, and the non-refundable fees will be forfeited.
  • Under the following conditions additional information may need to be provided to the Arizona Department of Insurance before the application can be approved:
    • there has been an administrative action imposed against an insurance license in any jurisdiction;
    • responded in the affirmative to a background question on the license application;
    • intended to operate under an assumed name, for which an assumed name certificate must be filed with the Department;
    • the application is missing information required by Arizona, or
    • the application appears to contain an entry that is not valid for Arizona.

Business Entity applicants must enter at least one ‘Individual’ entity type on the Owners, Officers, Partners and Directors (OPOD) page of the application.

Applicant may also need to register with the Arizona Corporation Commission or the Arizona Secretary of State to lawfully operate a business in Arizona. Other requirements for operating a business in Arizona may apply.

Applicants for the variable line of authority (LOA) must supply FINRA CRD number on application.

Applicant cannot apply for ADJUSTER-LMTD TO RES AUTH and Adjuster LOA on the same application

Business entity residents of DE, FL, IA, IL, KS, ME, MN, MO, NC, NE, SC, SD, TN, WI, WV and VT may only apply for the Business Authority = Individual Authority LOA along with the LOA of Surplus Lines Broker or the Excludes Surplus Lines Broker LOA. The applicant may not apply or hold ‘Surplus Lines Broker’ and ‘Excludes Surplus Lines Broker’ at the same time.
