Applicants must apply for the License Classes and Lines of Authorities they qualify for. If you apply for a License Class or for Lines of Authority that you do not qualify, the Department is unable to refund your fees. ARS 20-167.

Business Rules (Individual)

Applicant's birth date must match current PDB record for resident state (if PDB birth date is blank this is accepted).

Applicant's name must match current PDB record for resident state (if applicable)

Applicant must be 18 years old or older as determined from the applicant's Date of Birth.

Variable Life and Variable Annuity applicants - If you intend to sell both fixed and variable life/annuity products you must also hold a Life line of authority (LOA) in addition to Variable in Arizona.

P.O. Box or PMB (Private Mailbox) cannot be used for either residence or business address.

All instances of phone number must be a valid 10 digit phone number.

Life Settlement Broker applicants are required to hold the Life line of authority (LOA).

Applicants applying for Navigator or Certified Application Counselor must provide training certification from United States Department of Health and Human Services. to Insurance Licensing Section 2910 N. 44th St., Suite 210 Phoenix, AZ 85018-7269 or email to licensing@azinsurance.gov. Please include your NIPR Transaction Number.

Surplus Lines Broker Applicants - If you intend to sell non-surplus lines property and casualty products you must also hold the Property and/or Casualty lines of authority.

Applicants applying for Navigator must have at least one entry on the Affiliations section of the application that is actively licensed as a Navigator in Arizona.

Applicants with a status reason of surrendered cannot reapply for one year from date of surrender. After one year applicant must reapply as new.

Applicant's Non-Resident (active or inactive) AZ license must not have the following status reasons:

  • Suspended
  • Revoked
  • Deceased
  • Refuse to Renew
  • Military

If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.

If you intend to do business under a name other than your legal name (called an "assumed name" or "DBA name"), complete and submit Form L-193 (Certificate of Assumed Business Name).

Protect your assumed name by registering it as a trade name with the Arizona Secretary of State. If you do not register a trade name, another person may register the name, which will force you to stop using the name


Licensing Time Frames:

The Administrative Completeness Review Period (ACRP) for an application is one hundred and twenty (120) days pursuant to Arizona Revised Statutes Section 41-1071 et seq., and Arizona Administrative Code R20-6-708. The ACRP is the time within which the Arizona Department of Insurance must determine whether all required information has been submitted on the application. The ACRP is suspended if an application does not contain all the information required to be submitted. The online application may represent only a portion of information required for a complete license application. After submitting an electronic application, NIPR will send a confirmation e-mail informing that the license application has been accepted by Arizona or that additional information is required before the application can be accepted.

  • If the application is accepted by Arizona, the ACRP is deemed completed and the Department must make a substantive decision concerning the license application within sixty (60) days after the close of the ACRP. Generally, once a license application is accepted (and not deferred), a license is issued within a couple business days. Verify that a license has been issued by looking up information about the license on Arizona's state web site. The license certificate will be sent to the mailing address furnished on the application.
  • If additional information is required (for example, if the application is deferred), the ACRP will be immediately suspended, and the Insurance Licensing Section must receive a response to each specified deficiency or request for additional information within sixty (60) days after the date the e-mail was received from NIPR. Please include the Non-Resident License (NRL) transaction number in all written correspondence to the Department. If the Insurance Licensing Section does not receive a response to each specified deficiency or request for additional information within sixty (60) days, the application will be deemed withdrawn, the Department shall not be required to take any further action with respect to it, and the non-refundable fees will be forfeited.
  • Under the following conditions additional information may need to be provided to the Arizona Department of Insurance before the application can be approved:
    • there has been an administrative action imposed against an insurance license in any jurisdiction;
    • responded in the affirmative to a background question on the license application;
    • intended to operate under an assumed name, for which an assumed name certificate must be filed with the Department;
    • the application is missing information required by Arizona, or
    • the application appears to contain an entry that is not valid for Arizona.

Applicant cannot apply for Adjuster license class with Limited to Resident Authority Line of Authority (LOA) and Adjuster Line of Authority (LOA) on the same application.

If applicant actively holds Line of Authority (LOA) of Limited to Resident Authority on PDB do not allow applicant to apply for Adjuster Line of Authority (LOA).

If applicant actively holds Line of Authority (LOA) of Adjuster on PDB applicant cannot apply for Limited to Resident Authority.

Application for Adjuster line of authority will only be permitted if there are no active resident state adjuster licenses on PDB.

Application for Portable Electronics Adjuster line of authority will only be permitted if there are no active resident state adjuster licenses on PDB.

Application for Limited to Resident Authority should only be permitted if there is an active resident state adjuster license on PDB. *excludes public adjusters

If applicant actively holds Adjuster line of authority (LOA) on PDB, application for Portable Electronics Adjuster will be rejected.

Title applicants must send a letter of authorization from the insurer you represent to the Insurance Licensing Section ATTN: Electronic Licensing 2910 N. 44th St., Suite 210 Phoenix, AZ 85018-7269 or email to licensing@azinsurance.gov. Please include your NIPR Transaction Number

Exactly one Designated Responsible Licensed Producer (DRLP) must be provided on application who holds an active license in Arizona
