Add New Document

Enter the information for Reporting of Action and attach the document(s).

Indicator Definition
1 Category - Select the background question(s) you would like to attach a document to. To select multiple questions, press and hold the Ctrl key while selecting the questions with the mouse left key.
Note: Make sure to select question(s) for the right application. To view the question text, click the Help link (indicator 2 on the image above).
2 Help - Click to view the background questions text. A new window will display.
3 Date of Action - Date of Adjudication or the "date of final disposition of the matter" for administrative (and civil) actions, and the "date of initial pretrial hearing" for criminal actions per the NAIC Producer Licensing Model Act, Section 17.
Enter the date of action in mm-dd-yyyy format.
Note: The correct format is very important here: any other date format will return an error.
4 Document description by producer - Describe the document submitted.
5 Browse - Click to select a file from your computer.
6 Back - Click to go back to the Home screen without attaching the document
7 Attach Document - Click to attach the selected document.

Provide all required information, select a file, and click .

Note: File formats accepted: doc, docx, gif, jpg, jpeg, jpe, pdf.

You will be taken to the Attestation page.

Note: The document will not be saved if the payment is not made during this session. A session expires for non use (30 minutes without activity), or ends when you log out, the browser is closed, or the system errors out.

If you would like to add more documents, you can go ahead and pay the fee, and then you will be able to attach more documents for free before this session ends.
